Moving into care starts with a Needs Assessment. This assessment helps decide the right level of care (rest home, hospital level, dementia-specific). It’s not a test; it’s a practical way to match a person’s needs with the right services.
Talk to your GP, or contact NASC (Needs Assessment & Service Coordination) to request an assessment.
The assessor will review health, mobility, daily needs and social supports. Outcomes can include support at home, a recommendation for rest home care, or other community options.
Quick checks before you start

Paperwork
Bring medical notes, current medication lists and contact details for your GP and pharmacy.

Support people
Include a family member or trusted person for decisions. Have your Enduring Power of Attorney (EPOA) details ready if relevant.

Emergency needs
Tell the assessor about any recent hospital stays, falls, or urgent health concerns.
After deciding to move in — the next steps
Once the assessment recommends rest home care and you’ve chosen Glenwood, these practical steps below help make the move clear and simple.
1. Get your Assessment Certificate & finance form
Your NASC assessor will provide an Assessment Certificate and, if relevant, a Residential Care Subsidy application (financial means assessment).
2. Talk it through with family
Discuss options, read the Admission Agreement, and note any extra charges so everyone understands the costs.
3. Check vacancy reports
Use the Eldernet vacancy report to see real-time availability; we’ll also confirm our availability when you call.
4. Visit shortlisted homes
Schedule visits to Glenwood (ask for a sample room and a walkthrough of daily life and services).
5. Request or read the Admission Agreement
Ask for a copy of our Admission Agreement — check fees, inclusions and any optional service charges. Take time to read and ask questions.
6. Agree on a move-in date
Once you decide, we’ll arrange a pre-admission meeting, complete paperwork and plan your orientation and settling-in support.
Financial arrangements
Organise payments, apply for any subsidies, and confirm how bills and services will be managed.
